Chiropractor used my Forms without permission

I'm an independent contractor growing my practice inside a chiropractor's office. She just hired another therapist and gave them my custom forms...intake, SOAP, invoice, after covering my logo with hers. I'm addressing it, but I am want to know how others would handle this?

Yeah that's not cool at all. I'd bring it up direct and professional, but also consider copyrighting your forms moving forward. You gotta protect your work, especially when you're building your own brand.
